Choreoathetosis in Pediatric Patients
نویسنده
چکیده
Choreoathetosis (CHO) in pediatric patients following cardiopulmonary bypass (CPB) has no known etiology, although several causal factors have been suggested. The infrequent occurrence in any one institution and the inability to perform prospective studies have made the etiology of CHO difficult to discover. This paper discusses a retrospective analysis of eleven cases of CHO following CPB. To form a control group for matched comparison, each of the eleven CHO patients was matched with a patient who did not develop CHO. Matching parameters included: age at operation within 10%, diagnosis (cyanotic, not cyanotic), race, gender, operation, and date of operation within 12 months. Fifteen preoperative and CPB variables were evaluated to determine differences between the CHO patients and the control patients. Statistical analysis included odds ratios for matched pairs and two sample t-tests. A p value of 0.05 was chosen to assess statistical significance. Variables found to be significantly different between the study and control groups were: lowest rectal temperature, cooling and warming rates, and lowest arterial blood temperature. From these results, it is concluded that cooling to rectal temperatures less than 15°C or a cooling rate greater than 0.4 OC/min is associated with the development of CHO following CPB in these patients. Molecular diagnosis of pseudohypoparathyroidism type Ib in a family with presumed paroxysmal dyskinesia.
We describe 2 sisters diagnosed initially with paroxysmal kinesigenic choreoathetosis, a condition characterized by brief episodes of spasms precipitated by sudden movement. However, subsequent testing showed hypocalcemia, hyperphosphatemia, and elevated parathyroid hormone levels consistent with pseudohypoparathyroidism type Ib. This diagnosis was confirmed by genetic testing, which identified...
متن کاملRehabilitation for a child with recalcitrant anti-N-methyl-d-aspartate receptor encephalitis: case report and literature review
Anti-N-methyl-d-aspartate (anti-NMDA) receptor encephalitis is a newly recognized, potentially fatal, but treatable autoimmune disease. Good outcome predictors include milder severity of symptoms, no need for intensive care unit admission, early aggressive immunotherapy, and prompt tumor removal. We report a case of a young girl aged 3 years 2 months and diagnosed as recalcitrant anti-NMDA rece...
متن کاملParoxysmal Kinesiogenic Choreoathetosis: a Frequently Misdiagnosed Movement Disorder
Paroxysmal kinesiogenic choreoathetosis is rare movement disorder, which is frequently misdiagnosed as an epileptic disorder partly because of a good response to anti-epileptic drugs (Phenytoin sodium). At times these patients are mistaken for a conversion reaction. One such case is described.
متن کاملPost-streptococcal autoimmune disorders of the central nervous system.
PURPOSE OF REVIEW Autoimmune disease has long been intertwined with investigations of infectious causes. Antibodies that are formed against an infectious agent can, through the process of molecular mimicry, also recognize healthy cells. When this occurs, the immune system erroneously destroys the healthy cells causing autoimmune disease in addition to appropriately destroying the offending infe...
متن کاملThyrotoxic periodic paralysis and chorea: two uncommon neuromuscular complications in an adolescent with newly diagnosed Graves disease
Both thyrotoxic periodic paralysis (TPP) and choreoathetosis are unusual complications of childhood thyrotoxicosis. We hereby report a boy who presented with both TPP and choreoathetosis at the initial presentation of his Graves disease. A 14 year-old Thai boy presented with acute generalized proximal muscle weakness and myalgia for 5 hours. Detailed history revealed the increased appetite, hea...
